Case Management

The Home serves as the lead agency in the development, coordination, and provision of care for two case management systems sponsored by Commonwealth agencies. One serves the Department of Mental Health Boston region and one serves the Department of Children and Families (formerly the Department of Social Services) Park Street Area office.

Children's Community Support Collaborative
The Children's Community Collaborative (the "Collaborative") in Brighton organizes and manages a treatment continuum for youth with significant emotional and mental health needs and their families. The Home is the sole manager of the Collaborative’s system of care in the City of Boston. The Collaborative has a solution-focused, competency-based, managed care treatment model. It has assembled an integrated network of Boston providers, with a rich assortment of specialty and flexible services. The program also directly provides residential and home-based services. Other services are purchased from within The Home for Little Wanderers or from a wide range of area agencies, ideally in the client’s local community. The Collaborative stresses family participation in all phases of treatment.

Family Networks (Park Street Area Office)
As part of the Department of Children and Families (DCF) Family Networks initiative, The Home has been designated by DCF as the Area Lead Agency for the Park Street DCF Area Office Boston. In partnership with DCF, The Home develops and manages a comprehensive network of DCF contracted services and non-DCF community supports for children, adolescents, and their families, and provides case coordination through Family Team meetings. The goal of the network is to achieve permanency for DCF children and adolescents through family preservation, or by unifying or reunifying children and families. However, for those youth for whom permanency within a family setting is not possible, the goal is to ensure that they are prepared to live independently in the community. Network services include an array of family-based services, residential programs, group homes, independent living programs, specialized foster care, community supports, and other flexible support services.
