The Youth Permanent Connections program helps young people involved with the Department of Children and Families (DCF) build and sustain the lifelong relationships they need to thrive. Our team works alongside youth to identify caring adults from their past or present, strengthen existing connections and prepare both youth and families for meaningful reunification after time apart.
Using a youth-guided approach, we help young people explore who they trust, reconnect with supportive relatives or mentors and develop the skills and readiness needed for lasting family bonds. We also support families through the transition of welcoming a child back into their home, whether through reunification, guardianship or adoption.
The program continues to assist families after placement or after an adoption is finalized, offering guidance, therapeutic support and stability-building resources to help relationships grow stronger over time.
Youth Permanent Connections is about ensuring every young person has dependable, lifelong connections with adults who care about them—because no one should enter adulthood alone.
