At The Home, we believe strong families are the backbone of strong communities. Our Commuity Services Agencies (CSAs) take a team-based approach to providing individualized, family-driven care to meet the complex needs of children and families who are involved in child and family-serving systems and may be experiencing emotional, behavioral, safety or mental health difficulties. Our work brings together families, providers and key natural supports to create a plan of care that responds to the unique needs of each child and family.
The Home’s CSAs are two programs within a network of nearly 30 agencies across the Commonwealth that are part of the Massachusetts Children’s Behavioral Health Initiative (CBHI). Our CSAs serve youth from birth to age 21 who have experienced Serious Emotional Disturbance (SED), meet certain medical criteria and are enrolled in MassHealth Standard or CommonHealth.
Our Intensive Care Coordinators assess the strengths and needs of each family, and facilitate Care Planning Team meetings. Care coordinators identify resources and work with your family to assemble and synchronize a team of professionals, such as social workers, teachers and personal supports like friends and relatives, who work in unison to help you and your child achieve your goals.
Our Family Partners guide parents and caregivers, offering coaching as you work to meet your child’s needs. Family Partners have children of their own with special needs. Because of their “lived experience,” they understand what you and your family are going through, and will share tools to help you nagivate child and family services.
